Simply download the application and return it along with your investment check to the address shown on the application. All checks must be made payable to "Nasdaq-100 Index Fund". All purchases must be made in U.S. dollars and checks must be drawn on banks located in the U.S. (Third party checks, credit card checks and cash will not be accepted). |
